Community occupational therapy, centred on: OT assessment and intervention, functional capacity assessments (FCAs) and formal report writing, assistive technology and equipment, minor home modifications, and home safety and accessibility. Communio is a generalist practice, so if your situation involves daily life, independence or participation, it's worth asking — even if it's not on the list.
Anyone. You can refer yourself or a family member, and referrals are welcome from support coordinators, plan managers, GPs and other health professionals, and aged-care providers. You don't need a GP letter or any paperwork to start — the referral form on the contact page takes about two minutes and works for every funding type.
Occupational therapy is typically funded under Capacity Building — Improved Daily Living, billed at the current NDIS price guide rate. Self-managed and plan-managed participants can claim directly. You'll receive an itemised written quote before anything begins, so there are no surprises. If you're not sure how your plan is managed, it says so on the front page of your plan — or your support coordinator can confirm.
Yes — Communio is growing its work with older Australians, including home safety and accessibility, equipment, minor home modifications and staying independent at home. If you're registered with My Aged Care or have a home care provider, mention it in your referral and we'll work out the right pathway together. If you're not registered yet, we can point you to the right starting place.
That depends on your funding. NDIS work is billed at the NDIS price guide rate; aged-care and private fees are quoted upfront at the same hourly basis. Whatever your situation, you'll always receive a written, itemised quote before any work starts — no hidden costs, no surprise invoices, and honest advice if a cheaper or better-fit pathway exists.
Wherever life happens. Communio is a community-based practice, so sessions usually happen in your home or out in the community — because that's where the real picture is. Telehealth is available where it's clinically suitable, and we'll always tell you honestly if an in-person visit would give a better result.
The form sends your details straight to the Communio inbox — nowhere else — and you'll get a reply within two business days. It asks only for what's needed to respond sensibly: who you are, how to reach you, your funding type (NDIS, My Aged Care, private, or not sure) and a sentence or two about the situation. Nothing is shared with anyone without your consent.
Just send the referral anyway and choose "Not sure yet" for funding. Working out the right pathway is part of the first conversation — and if Communio isn't the right service, you'll get an honest answer and a suggestion of where to go next, free of charge.
